This long-awaited revised edition of Prof. Francesco Furlanello’s time-honored compilation of antiarrhythmic regimens reflects new insights into current concepts in that most delicate subspecialty of cardiology. In its bewildering complexity, the management of cardiac arrhythmias continues to be a daunting challenge. Tremendous advances in diagnostic technologies designed and developed to unravel the pathophysiology of cardiac dysrhythmias have – paradoxically enough – revealed that the mechanisms of arrhythmogenesis and various associated bioelectrical phenomena (e.g., electrical remodeling or moving target sites) are exceedingly intricate and indeed elusive, making the search for the most appropriate therapeutic approach a disheartening endeavor at times. Unfortunately, the pharmaceutical players in the field have been marking time in recent years. No magic bullet has been forthcoming for any arrhythmologic indication, and, with few exceptions (as rightly observed by the authors in their Preface), research has been on the back burner for too long: Not a single new drug has become available for general use in years, and only a handful are undergoing clinical testing.
