										<content:encoded><![CDATA[<p>L’elastosonografia è una metodica ecografica di recente acquisizione che si propone di discriminare il differente grado di elasticità propria dei tessuti molli. Introdotta per la prima volta nel 1991, tale metodica in base al diverso grado di rifrazione subito dal fascio ultrasonoro, a seguito di pressioni progressive esercitate dall’operatore con il trasduttore sulle strutture in esame, consente oggi di visualizzare in real-time, tramite una finestra colore sovrapposta all’immagine in B-mode, l’elastogramma di riferimento. Diversi studi hanno già dimostrato l’attendibilità della elastografia applicata alla risonanza magnetica per lo studio della risposta muscolare in relazione al tipo di contrazione; poiché il segnale in RM è fortemente correlato alla composizione istochimica dei tessuti, sulla base della diversa densità protonica delle varie componenti muscolari in relazione alla differente distribuzione del tipo di fibra muscolare costituente la struttura muscolo-tendinea, è possibile ottenere differenti segnali nelle sequenze T1 e T2 pesate, caratteristici del momento funzionale in esame. Tuttavia è bene ricordare come l’esame di RM, pur consentendo una notevole panoramicità delle strutture esaminate, resti sempre e comunque un esame statico che non consente una visualizzazione in tempo reale delle componenti muscolo-scheletriche in esame. L’ecografia è universalmente riconosciuta come metodica di prima istanza nello studio di muscoli e tendini; è altresì noto come uno dei problemi più diffusi dell’esame ecografico standard sia la valutazione qualitativa delle strutture muscolo-tendinee esaminate, sia in condizioni fisiologiche che nella prognosi dei traumi muscolari laddove, a cicatrizzazione avvenuta, vi sia la necessità di stabilire il momento più opportuno per la ripresa dell’attività; non sempre è possibile infatti ottenere informazioni discriminanti in tal senso a carico dell’ecostruttura dei tessuti esplorati ed è inoltre esperienza comune come spesso, ad una struttura apparentemente integra, non corrisponda un equivalente di integrità funzionale. Il nostro studio sull’elastosonografia applicata all’apparato muscolo-scheletrico è partito da una valutazione puramente funzionale, basata sulla conoscenza della fisiologia e biomeccanica muscolare. In accordo con tali principi, ponendo come assodato che il massimo coefficiente elastico sviluppato da un muscolo sia rilevabile durante la contrazione, grazie ai meccanismi propri di autoregolazione meccanica, l’interpretazione della mappa colorimetrica ha permesso di confermare le attese secondo le quali i colori dell’elastogramma, indicativi di una maggiore elasticità intrinseca, sono più evidenti in condizioni dinamiche e più ancora in condizioni di carico in ortostatismo per ciò che concerne l’arto inferiore. L’esame ecografico è universalmente riconosciuto esame di primo livello nello studio dell’apparato muscolo-scheletrico, grazie soprattutto alla valutazione dinamica delle componenti muscolari. La possibilità di apprezzarne anche il grado di elasticità, rappresenta un ulteriore completamento dello studio ecografico tradizionale, anche in termini di valutazione preventiva all’inizio di una qualsiasi programmazione agonistica, consentendo degli allenamenti mirati atleta per atleta, nell’intento di scongiurare il rischio di traumi che spesso occorrono durante le fase di ripresa dell’attività sportiva, sia essa dopo un trauma, che dopo le soste stagionali. Lo studio mediante elastosonografia si pone quindi anche come valido supporto nel follow-up clinico-terapeutico delle lesioni muscolari, permettendo una più corretta valutazione del recupero funzionale in rapporto alla reale condizione delle fibre muscolari interessate dai processi riparativi. Riteniamo che questo volume possa essere di aiuto per tutti gli operatori del settore che intendano approfondire le proprie conoscenze in una materia, la diagnostica per immagini, che in questi ultimi anni ha assistito ad un notevole incremento delle proprie potenzialità diagnostiche grazie ai progressi tecnologici che oggi consentono di usufruire di apparecchiature sempre più complete e sofisticate, che meritano pertanto, un costante aggiornamento da parte di tutti gli utilizzatori in campo </p>

										<content:encoded><![CDATA[<p>Tanto devo nella mia carriera ad alcuni “maestri” che mi hanno formato nella crescita professionale. Il primo fra tutti è mio padre Carlo, che è stato Primario Radiologo al “mitico” Istituto Ortopedico Rizzoli, e che mi ha trasmesso i fondamenti della radiologia. Frequentando il Rizzoli e l’ambiente ortopedico, vissuto peraltro da me in prima persona in pronto soccorso per circa 2 anni, ho iniziato ad innamorarmi della diagnostica per immagini in campo muscolo-scheletrico. Va doverosamente citato il nonno Gian Giuseppe Palmieri, uno dei pionieri e capostipiti della Radiologia Italiana, cattedratico in Bologna, fra gli anni 30’ e 60’, e Presidente Onorario di innumerevoli Società Radiologiche Mondiali, che tanto ha dato alla Scuola Radiologica Italiana ed in particolare a quella Bolognese. Un doveroso ringraziamento va al mio Direttore di specialità, Professore Alberto Corinaldesi, ed al caro amico e cugino Gian Carlo Palmieri, con cui ho collaborato per tanti anni. In fine, e non per questo ultimo, un grandissimo riconoscimento deve essere rivolto al compianto Professor Turno Lubich, che con la sua fantastica lungimiranza, mi ha aperto la strada all’affascinante mondo della medicina sportiva. Il mio personale interesse verso questa disciplina risale a cavallo fra gli anni 70’ e 80’, quando, quasi da pioniere (ricordo che eravamo l’amico Giuseppe Balconi ed io in Italia e l’amico Bruno Fornage in Francia), cominciai ad effettuare studi ecografici sui cadaveri, onde poter perfezionare le conoscenze di anatomia, con le immagini partorite dalle prime apparecchiature compound manuali. Iniziammo le prime pubblicazioni e nel breve volgere di tempo si sviluppò a livello nazionale un notevolissimo interesse verso questo settore, che mi indusse ad organizzare nel 1985 un Congresso Nazionale di Ecografia Muscolo-scheletrica che radunò tutti i maggiori esperti italiani. In quel periodo ero aiuto co-responsabile del servizio di Radiologia presso l’Arcispedale S.Maria Nuova di Reggio Emilia, e Direttore del reparto di ecografia. Nel breve volgere di tempo mi trovai sommerso da richieste di medici che volevano frequentare il reparto, e fui costretto a ragionare sulla ipotesi di istituire una scuola apposita. Scuola che mi fu riconosciuta nello stesso 1985 dal Prof. Ludovico Dalla Palma, allora Presidente della Società Italiana di Radiologia, nell’ambito della Società Italiana di Ultrasonologia in Medicina e Biologia (S.I.U.M.B.). Nello stesso anno il Prof. Turno Lubich, allora Direttore della Scuola di Specialità di Medicina dello Sport all’Università degli Studi di Bologna, mi conferì la docenza di Professore a contratto in riferimento alla diagnostica ecografica dell’apparato Muscolo-Scheletrico, e la consulenza (a tutt’oggi mantenuta), presso lo stesso Istituto. L’interesse per questa dottrina continuava a crescere a dismisura, al punto che, nel 1989, grazie al fondamentale apporto del Prof. Roberto Passariello, fu istituita la sezione di diagnostica per immagini in Medicina dello Sport, che vantava come Presidente il “mitico” Prof. Folco Rossi, Vice Presidente il Prof. Antonio Francone, e Consiglieri Carlo Faletti, Fosco De Paolis, Ugo Albisinni ed il sottoscritto, e come Segretario Antonio Barile. Nel 1990, sempre grazie all’apporto del Prof. Turno Lubich, fui incaricato come ecografista ufficiale ai campionati mondiali di calcio “Italia 90”, esperienza che risultò esaltante. Nel 1992 assunsi la direzione del Dipartimento di Diagnostica per Immagini presso gli attuali Ospedali Riuniti Associati Nigrisoli, di cui tuttora sono Direttore. Sempre nello stesso anno fondai la scuola di Ecografia Muscolo-Scheletrica, che tuttora dirigo insieme all’ormai inseparabile amico e preziosissimo collaboratore Paolo Minafra. In quegli anni cominciò la mia intensa attività scientifica, durante la quale ho partorito svariati libri di ecografia muscolo-scheletrica e di imaging integrato. Sempre nello stesso periodo iniziò una assidua collaborazione e profonda amicizia con il gruppo dei radiologi americani costituenti la Società di Ultrasonografia Internazionale “Musoc”, con particolare riferimento agli amici carissimi Tony Bouffard e Marnix Van Holsbeeck, dirigenti radiologi presso l’Henry Ford Hospital di Detroit, presso il quale ospedale ho effettuato con loro uno stage. Questa proficua collaborazione mi ha consentito di essere relatore a numerosi Congressi internazionali, e nel 2004 mi è stato affidato l’onere, ma soprattutto l’onore, di organizzare il Congresso Mondiale di Ecografia Muscolo-Scheletrica a Bologna. Un’altra esperienza indimenticabile è stata la partecipazione alle Olimpiadi Invernali di Torino nel 2006 ed alle Paralimpiadi al Sestriere, in qualità di radiologo costituente la equipe capitanata dall’amico fraterno Carlo Faletti. In questi ultimi anni sono stato invitato, tramite lo staff di Esaote, a tenere svariate lezioni inerenti l’impiego del G-scan, la Risonanza Magnetica che effettua indagini della colonna e delle articolazioni in clino ed ortostatismo, presso svariate Università nel mondo fra le quali Cina, Argentina, Colombia, Messico e Spagna. Nel frattempo la mia scuola si è ulteriormente incrementata, e con mia grande soddisfazione è stata scelta ed inserita dal Comitato Internazionale ESSR, nel circuito mondiale di scuole e corsi di diagnostica muscolo-scheletrica. Una recente entusiasmante esperienza, è stata quella di partecipare ad un Cadaver-lab presso l’Istituto di Anatomia Umana dell’Università di Graz in Austria, insieme all’inseparabile Paolo Minafra ed all’amico ortopedico Luigi Romano, grazie al quale abbiamo potuto effettuare i preparati anatomici che potrete osservare nel libro. Un ringraziamento particolare va rivolto al Professore Friederick Andehuber, Direttore dell’Istituto stesso, grazie alla cortesia del Quale ci è stato concesso un accesso privilegiato a questi corsi. La mia professione di radiologo si è peraltro arricchita in tutti questi anni, ed in questo ultimo periodo in particolare, grazie alla collaborazione con l’amico Professor Claudio Tentoni, Direttore della Scuola di Specialità della Facoltà di Scienze Motorie presso l’ Università degli Studi di Bologna. La collaborazione costante con Claudio in Facoltà, mi ha permesso di sviluppare numerosi studi di cinesiologia applicati alla diagnostica per immagini, con particolare riferimento alla Risonanza Magnetica in ortostatismo (G-Scan) ed alla Elastosonografia.La più recente tappa della mia carriera è costituita dall’incarico conferitomi di Direttore del Centro Medico presso l’Autodromo Internazionale di Imola, nel quale, insieme alla mia equipe, presto assistenza in tutte le manifestazioni automobilistiche e motociclistiche nazionali ed internazionali. In riferimento a quanto sopraesposto, appare evidente con quanto entusiasmo abbia in questi anni affrontato il problema della diagnostica muscolo scheletrica. Mi auguro di potere trasferire tutte le esperienze maturate nella mia carriera, in questo primo volume di Tecnica ed Anatomia Ecografia e di Imaging integrato, che presto sarà seguito dal secondo volume contenente la patologia, e spero possa risultare di aiuto per tutti coloro che si apprestano ad intraprendere e sviluppare la metodica ecografica e l’imaging integrato in campo muscolo-scheletrico. </p>

										<content:encoded><![CDATA[<p>Negli animali d&#8217;affezione una delle tecniche di diagnostica per immagini utilizzata più frequentemente è la radiologia, data anche la diffusione degli apparecchi radiologici ormai presenti in quasi tutte le strutture veterinarie, anche le meno attrezzate.   Nel corso degli studi universitari però lo studio della radiologia purtroppo non è in grado di fornire l&#8217;abilità tecnica e la capacità interpretativa necessarie a gestire correttamente l&#8217;esecuzione di una lastra e a comprenderne il significato, una volta posta sul negativoscopio. Tutto ciò fondamentalmente per tre motivi .   Il primo è che, per le norme di radioprotezione necessarie a tutelare la popolazione dagli effetti negativi delle radiazioni, è molto difficile che gli studenti o i neolaureati siano ammessi nella sala radiologica durante l&#8217;esecuzione delle radiografie, e viene così a mancare tutto il bagaglio di informazioni necessarie (compresi i trucchi ) che serve a eseguire un radiogramma leggibile.   Il secondo è che, per poter correttamente apprendere sia l&#8217;anatomia radiografica normale sia quella patologica, sono necessari parecchi anni di esperienza e migliaia di radiogrammi visti e studiati, situazione impossibile durante il corso di studi universitari.   Il terzo, di rilevanza non indifferente, è la variabilità dei nostri pazienti che vanno dai canarini ai criceti, dai gattini neonati ai cani di taglia gigante, fatto che rende difficile stabilire i parametri di esecuzione che variano moltissimo. Spesso poi il tutto si associa alla scarsa collaborazione (se non alla resistenza armata) dei nostri pazienti, che trova una certa similitudine solo nella radiologia pediatrica dove il bambino raramente mantiene l&#8217;immobilità necessaria allo scatto perfetto. Per fortuna dei radiologi umani però è difficile che il bambino gli conficchi una dozzina di denti aguzzi che non mollano la presa su una mano. E questa non è una differenza da poco.  </p>

										<content:encoded><![CDATA[<p>Con questo testo, Gian Marco Giuseppetti, Pietro Panizza e Gianluca Valeri nelle vesti di autori, ma soprattutto di ideatori e coordinatori, hanno raccolto le esperienze della radiologia italiana in tema di risonanza magnetica della mammella, provenienti da centri, universitari ed ospedalieri, a più matura e consolidata conoscenza.  Leggendo queste Istruzioni per l&#8217;uso sulla MRM, immediatamente si notano le finalità del libro: non aprire una finestra sul futuro della metodica, che sarà sicuramente entusiasmante e ricco di novità, ma puntualizzare il suo stato dell&#8217;arte, fornendo quelle raccomandazioni e quei suggerimenti in grado di ottimizzarne i molteplici aspetti, siano essi tecnico-metodologici oppure più squisitamente clinici.  Tutto ciò in un testo semplice e di facile consultazione, arricchito da un aggiornato ed utile elenco di letture consigliate.  </p>

Cardarelli&#8221; Napoli  Descrizione  Il libro è focalizzato principalmente sulle applicazioni e sui reperti diagnostici della tomografia computerizzata del cranio e della colonna vertebrale, alla luce dei recenti progressi della tecnologia che hanno portato ad un drammatico miglioramento della qualità delle immagini TC dell&#8217;encefalo, dello scheletro facciale e della colonna vertebrale. Le enormi possibilità diagnostiche della Risonanza Magnetica non hanno ridotto l&#8217;interesse per la TC, che comunque rimane la procedura diagnostica iniziale; in ogni caso la correlazione con la Risonanza Magnetica p evidente in tutto il testo, anche quando non è specificamente citata. Pertanto il libro costituisce un riferimento esaustivo, in cui viene sempre sottolineato il ruolo complementare delle varie metodiche d&#8217;imaging (radiologia convenzionale, TC e RM). L&#8217;organizzazione molto schematica del testo, il riferimento continuo alla tecnica d&#8217;esame ed alla diagnosi differenziale lo rendono utile sia al radiologo non particolarmente esperto di neuroradiologia, sia al tecnico di radiologia, sia anche al neuroradiologo come strumento di consultazione rapida, ad esempio nella diagnosi differenziale dei tumori cerebrali. 										<content:encoded><![CDATA[<p>La storia del Laboratorio per Immagini degli Istituti Ortopedici Rizzoli di Bologna rappresenta un prezioso arricchimento per l’Ortopedia medica. La fase preparatoria del lavoro è stata indubbiamente molto laboriosa e la scelta del materiale da utilizzare &#8211; composto da reperti cartacei e da interessantissimi filmati d’epoca &#8211; veramente difficile, tanto ricco è il patrimonio iconografico storico dell’Istituto. Ma ne è valsa decisamente la pena. Alla fine, infatti, il risultato ottenuto è di ottima qualità. Attraverso le immagini e le parole il lettore potrà ripercorrere il cammino intrapreso alla fine dell’ottocento dalla Radiologia dell’Istituto Rizzoli, di fondamentale importanza per lo sviluppo dell’ortopedia. </p>

Se si scorrono i libri storico-celebrativi usciti nel corso degli ultimi mesi, si nota che il progresso della Radiologia è ascritto ad un gruppo numeroso di persone, ma non così elevato come l’importanza della disciplina farebbe supporre. Il dizionario del dottor Bosnev ricorda tutti gli studiosi che non hanno trovato posto nella &#8220;nomenklatura&#8221; radiologica.
